North Point Pediatrics offers online access to patient forms in order to help our patients save time by filling out forms before their office visits.

Transferring Records TO Another Practice
When requesting your child(ren)’s medical records to be transferred to another physicians office, please be advised that the required time needed to complete your request is 2 weeks. In the event you are requesting copies of the medical records yourself, there will be a $25 fee per child in accordance with Georgia Statute 31-33-3. We will make every effort to complete your request in the shortest time possible.
Forms Policy
Due to high demand for physical and immunization forms, standard processing takes 3-5 business days to complete.
Form Processing Options:
- Standard processing: $10 per form (3-5 business days)
- Expedited processing: $25 per form (completed within 24 hours)
We will do our best to get your forms to you as soon as possible. Thank you for understanding.
Forms requiring $10 fee:
- Sports Physical Forms
- College Health Forms
- Camp Forms
Katie Beckett forms require a $25 fee due to their complexity.
Once completed, we can have the forms available for you to pick up at the office, we can upload them to your child’s portal or we can return the forms to you by mail or by fax.
