Office Policies

Appointments & Cancellations

We make every effort to honor all time commitments and request that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. 24 hours notice is required to cancel a well visit appt to avoid being charged a $50 cancellation fee. These appointments are booked months in advance and can't be filled if they are cancelled the same day.

Prescription Renewals

Prescriptions will only be refilled during regular office hours. The on-call physician will not prescribe or refill any medication after office hours. When requesting a refill, please use the patient portal for faster service. Allow 5 business days for us to complete your request. We will make every effort to handle your request in the shortest time possible. Provide the name and the address of your pharmacy along with the name of the medication, dosage and quantity. No prescriptions for any scheduled medication such as narcotics, Ritalin or Adderall will be renewed over the phone.

Insurance & Payment Policy

If you are insured by one of the plans in which we are a participating provider, we will gladly follow the contractual arrangements. You must have your plan ID card at the time of each visit and must be prepared to pay your co-pay, deductible or any services that are not covered by your insurance. We require payment at the time of service. We are pleased to accept cash, check, MasterCard, Visa or Discover for your convenience. If you are on a plan for which we are not providers, our business office will be happy to provide you with the necessary paperwork for you to file with your insurance company. Our business office is available to answer any questions you may have regarding your account or insurance coverage.

Records Transfer

When requesting your child(ren)'s medical records to be transferred to another physicians office, you will need to sign a release of information form. Please be advised that the required time to complete your request is 2 weeks. In the event you are requesting copies of the medical records yourself, there will be a fee per child in accordance with Georgia Statute 31-33-3. We will make every effort to complete your request in the shortest time possible.

School & Camp Forms

Due to the overwhelming demand for camp, physical, immunization forms, etc; we require 5 to 7 business days to complete the forms. Please plan ahead. We will do our best to get your forms back to you as soon as possible. Thank you for understanding.

Once completed, we can upload the forms to your child's portal, have the forms available for you to pick up at the office or return the forms to you by mail or by fax. Forms can't be emailed due to HIPPA guidelines.

The following forms require a payment of $10 per form at the time of the request. You do have the option to have the form expedited for a fee of $25 We will get you the form within 2 business days.
