Financial Policy
We are committed to partnering with you to provide your child with the best medical care possible. As with any other partnership, there are expectations by both parties. We want to make sure that our epectations are clear to prevent any misunderstandings.
- You need to know and understand your own benefits
- You need to bring your insurance card to each visit
- You will need to pay your copay, co-insurance, deductible or any outstanding balance at the time of service (including telehealth visits)
- You will need to pay in full at the time of service, if we aren't able to verify your coverage at the time of service
- We offer a prompt payment discount for "self pay" patients
- We do not file secondary insurance claims, but will provide you with a copy of the charges for you to file with your secondary insurance carrier
- You will be charged a $50 "no show" fee for well checks or consultation appointments if you fail to provide 24 hours notice
- Patients with outstanding balances greater than 60 days must make arrangements for payment prior to scheduling appointments.

Divorce/Separation & Custody Agreements: The attending parent will be responsible for copayments and deductibles at the time of service. Copies of these charges are available at the request of the attending parent.

- BCBS PPO, HMO, POS. OAP (Group Plans)
- Beechstreet
- Cigna HMO, PPO, OAP/ Cigna International
- Cigna + Oscar
- Coventry/Southcare/First Health (National, Leased, HMO) - No out-of-state plans
- Great West
- Humana (Choice Care Network)
- Kaiser through PHCS (group plans only thru PHCS - no HMO group and no individual plans
- Lifewell Health Partners
- Medical Partners of America (MAG Mutual)
- Multi Plan
- PHCS
- Tricare East
- Unicare
- Liberty Health Share
- UHC
If you do not see your insurance plan listed, please call our office. We are working on becoming a participating provider with many of our community’s most popular health plans.
The managed care health insurance market changes rapidly, and specific coverage questions should be discussed directly with your insurance company.
Insurance Plans Not Accepted
We do not accept the following insurance plans:
- Ambetter
- Amerigroup
- Kaiser (INDIVIDUAL PLANS or HMO Group)
- Medicaid
- Peachstate
- Tricare/Champus
- United Healthcare COMPASS
- United Healthcare "Core Network"
